Perhaps it’s the everyday things that take so much effort. Perhaps you’re trying to explain what your child finds difficult. You don’t need to have it all worked out before you start.

MEET YOUR ADHD PASSPORT

Your screening findings.
Reviewed and explained.

Your ADHD Passport includes a clinical summary, detailed questionnaire results and an introduction letter. It brings together your account and available informant contributions, with clinician review before release.

  • A clinical summary. The main findings, their everyday impact and whether further formal assessment is recommended.
  • Informant perspectives. Contributions from someone who knows you well, or a teacher for a child, alongside your own account.
  • Detailed results and visual profiles. Each questionnaire explained, with comparisons where matching information is available.
  • Yours to share. With your GP, school, SENCo, university, workplace or assessment provider.
  • Authenticity you can check. Organisational digital certification and a QR link to verify the issued document.

A screening and pre-assessment tool, not a diagnosis.

INFORMATION FROM PEOPLE WHO KNOW YOU

Include their perspective.

For adults, someone who knows you now and someone who knew you in childhood can add useful context. For children, a teacher’s observations help show how things compare at home and school.

You can see what is still outstanding and choose to wait before buying your Passport. We notify you when a contribution arrives. If information is unavailable, that is made clear in the report.

BEFORE YOU START

Your questions,
answered.

What is free, and what costs £99?

The full screening questionnaires are free to complete. Your answers are saved and you can review the information you provided. The optional £99 ADHD Passport adds the screening findings, explanations, clinician review and introduction letter. Your own Passport is prepared only after purchase.

Is this for adults or children?

Both. There is an adult pathway for people aged 18 and over and a parent-led pathway for children aged 6 to 17. You choose the appropriate pathway when you start.

Will it tell me whether I have ADHD?

It is screening, not diagnosis. Your reviewed Passport can identify screening thresholds and areas to explore, and recommend whether further formal ADHD assessment is warranted. A diagnosis requires a comprehensive assessment by an appropriately qualified clinician.

Can I stop and come back?

Yes. Your progress is saved so you can return to where you left off. Unpaid screening information is kept for 90 days, with reminders before deletion. Your dashboard shows the date.

What if I’m waiting for someone else’s answers?

You can see which supporting contributions have arrived and choose to wait before proceeding. We let you know when a contribution is received. Missing information is made clear so you can decide how to continue.

Can I use my Passport with my GP, school or employer?

Yes, it is yours to share as a starting point for discussion. It helps explain the information you have provided and the screening findings. The recipient decides what further evidence, assessment or support is appropriate.

Who can see my information?

Your screening information is private and access is controlled. A clinician reviews your paid Passport before release. You decide who to share your downloaded Passport with. Read how we use and protect your information.
