What the screening covers

Record symptoms, history and everyday impact.

The screening records current concerns alongside relevant history and examples from daily life. Information from different people or settings remains clearly labelled in the report.

1

Describe your current experiences

Record how attention, organisation, follow-through and restlessness affect work, study, home life and relationships.

2

Add relevant childhood history

Structured questions help you record when concerns were first noticed and how they appeared at different stages of life.

3

Record mood, anxiety and daily functioning

Mood, anxiety and clearly labelled executive-function responses provide context alongside the ADHD screener.

Questionnaires and scoring

See the screening tools and scores included.

The report names each tool and shows its score with the relevant published band or documented scoring rule. Personal history and non-normed questions are clearly labelled as context rather than clinical thresholds.

ASRS-v1.1A recognised adult ADHD screener, shown with its score and published interpretation.
GAD-7Anxiety symptoms shown in their published band, keeping overlapping experiences in view.
PHQ-9Mood symptoms shown in their published band and considered alongside the attention pattern.
Structured contextEarlier experiences, day-to-day impact and clearly labelled executive-function questions.
