How to use the Passport

  1. Agree what the client wants to share

    Explain how any document will be used and stored within your own service before accepting it.

  2. Use it as context, not a formulation

    Explore the person's experience through your own assessment. The summary may identify questions to discuss, but does not replace your professional judgement.

  3. Connect the findings to the client's goals

    Discuss what may help them engage with therapy and whether further assessment would be useful alongside that work.

What the Passport does not decide

How review and document verification work

The person chooses who receives their copy. Agree a secure way to share it and handle it under your organisation's confidentiality and retention procedures. The verification page does not disclose their questionnaire answers or clinical findings.
