A child ADHDassess report presents parent ratings, optional teacher input, developmental information and reported impact. It can contribute to school discussions without replacing the school’s own assessment and evidence.
Built for the graduated approach
The report keeps parent and, where available, teacher ratings separate, alongside structured developmental information and examples of daily impact. Schools can consider this information within the “assess, plan, do, review” cycle, together with their own observations and records. It does not replace an educational psychologist’s assessment.
For an EHCP needs assessment request
- A one-page EHCP support letter addressed to the local authority SEND team.
- Structured information from home and, where supplied, school to sit alongside wider SEND evidence.
- A named, qualified clinician reviews and signs the evidence.
- A clear statement that screening sits alongside, not instead of, EP and statutory assessment.
How a family may use the report
A family may share the report while waiting for CAMHS, community paediatrics or another assessment service. It gives the school structured information to consider alongside current SEN Support records and may also be shared with a later assessing clinician.
