ADHD screening reports for GP referral conversations.

An ADHDassess report presents scored screening questionnaires, relevant history, reported impact and limitations in one document. A focused support letter summarises why the person is asking you to consider referral.

What you receive

The person can share a detailed, clinician-reviewed screening report and a one-page referral support letter. The letter summarises the screening findings, refers to relevant NICE NG87 recommendations and asks you to consider referral for a full diagnostic assessment. The reviewer’s professional details are shown on the final document.

What can be reviewed during an appointment

  • Published instruments, scored against published thresholds, not self-interpreted scores.
  • Parallel screening of mood, anxiety and sleep, so differentials are already considered.
  • A concise referral question that can be considered alongside your clinical judgement.
  • For patients in England, the letter can name the Right to Choose route, where the patient has expressed that preference.

What it is, and is not

ADHDassess is a screening service, not a diagnostic assessment, and it does not prescribe. It is designed to support, not replace, your clinical judgement and the assessing service’s work. You remain the referrer, and the decision is yours.
