How to use the Passport
- Ask the patient what they want help with
Their priority may be concentration at work, parenting, study or understanding longstanding difficulties. Start there, then use the summary to structure the discussion.
- Review the evidence and its limitations
Check the date, sources and informant coverage. The introduction letter and clinical summary provide a starting point; the complete Passport supplies the detail.
- Agree the next step using your clinical judgement
Consider local referral criteria and other relevant explanations or needs. The patient can retain the PDF and share it with a subsequent assessment provider.

What the Passport does not decide
The Passport is screening evidence, not an ADHD diagnosis, a prescribing recommendation or a request to accept shared care. Referral, diagnosis and treatment decisions remain with the relevant clinicians.
