Your information is brought together
Screening results, relevant history and available informant accounts are organised into a draft. Automated scoring and AI-assisted drafting support preparation; they do not make a diagnosis.
A useful health document needs more than a professional appearance. Your Passport has clinical review, an identifiable organisation behind it and a way for recipients to check the issued document.

Screening results, relevant history and available informant accounts are organised into a draft. Automated scoring and AI-assisted drafting support preparation; they do not make a diagnosis.
The clinician checks the findings, interpretation and limits of the available evidence before approving the document for release. Missing contributions should be visible, not concealed.
The introduction letter and full report form one PDF. Neuro FX Ltd applies its organisational digital certification to the issued document, with a timestamp and verification reference.
Check that the address belongs to ADHDassess.com. Enter the document reference and the person's matching details from the Passport. There is no public search by name.
A successful match shows the document's issue date, revision and status. It does not disclose questionnaire answers, scores or clinical findings.
The page can compare the file's digital fingerprint with the issued copy. That fingerprint is calculated on your device; the PDF is not uploaded for this check. Adobe Acrobat can also validate the embedded organisational signature.
It establishes a match with an issued document or verifies the digital signature, depending on the check used. It does not independently prove the reader's identity, validate every statement made by an informant or turn screening into a diagnosis.
Saving, printing to PDF or editing metadata can change a file's fingerprint. A mismatch needs investigation, not an accusation. Ask for the original downloaded PDF. Content-based checking is only enabled after its validation; it is not a substitute for a valid signature.
