How to use an ADHD screening report with your GP
A GP appointment is easier to use well when you can state the main problem, give concrete examples and make a clear request.
The short answer
Send or bring the report, summarise the main functional difficulties and ask whether referral for specialist ADHD assessment is clinically appropriate. Say if you want to discuss an NHS local route, Right to Choose in England or a private referral. Your GP makes the referral decision and may need its own forms or information.
Lead with the effect on daily life
Explain what is difficult and why it matters now. Two or three clear examples are more useful than reading every questionnaire result aloud.
For an adult, this might include repeated work errors despite checks, missed bills or a pattern that has affected education and relationships since childhood. For a child, describe the support needed at home and school and any effect on learning, friendships or emotional wellbeing.
Make a clear request
You can ask: "Could we discuss whether referral for a specialist ADHD assessment is appropriate?"
If you live in England and want to consider Right to Choose, name the provider you are considering and bring its current referral instructions. Right to Choose is not self-referral. NHS England says the referrer remains responsible for clinical appropriateness and the provider must meet the relevant NHS contract conditions.
Expect local requirements
The GP may ask questions about childhood, mental health, physical health, substance use or other possible explanations. A local service may require its own form, school evidence or triage process.
Your ADHDassess report is supporting information. It does not oblige a GP to refer or a provider to accept the referral.
If referral is not agreed
Ask what information is missing, what alternative explanation should be explored and what support is available for the difficulties you described.
